KINSHIP EMERGENCY INFORMATION FORM

Use this proactive form to provide the county with information about your desire to be a kinship caregiver in the event of an emergency placement of your grandchild or relative child.

The following counties are currenlty piloting this new program: Sherbourne, Olmsted, Carver and Dakota.

The Kinship Emergency Information Form (KEIF) is a document that grandparents or other relatives can fill out and give to their county social service department. In the event that there is an emergancy placement of the child, county officials can refer to the KEIF in order to find a placement home.

Any relative can complete and submit the KEIF if they are available to act as a temporary or permanent kinhsip caregiver for the child(ren).
