FORMS:

Motion to Intervene Packet. This packet provides instructions and documents to file with the court in order to become a party in a child protection case.

Motion to Intervene as a Matter of Right (grandparent) Packet. If you are a grandparent of the child in a juvenile protection matter, and the child has lived with you during the two years prior to the filing of the protection petition, you may automatically become a party to the proceedings.

CHIPS Petition (Private/Pro Se) Forms. If you feel a child is being abused or neglected and is in need of protection, please contact the social service agency where the child(ren) resides before filing this peition.

Fee Waiver/In Forma Pauperis Forms. If you are petitioning the court and your income/asset level is low enough, it is possible to have your court fees (filing, expert witness, motions) waived. You must submit these waiver forms at the time you file your petition to the court.
